Feel It, Name It | Bodily Sensation Mapping – A Scalable VR Application for Accessible Mental Healthcare

Ein VR-basiertes, körperorientiertes Tool soll Menschen dabei helfen, Emotionen besser wahrzunehmen, zu benennen und zu regulieren – als zugängliche Ergänzung in der psychischen Gesundheitsversorgung.

Abstract

Emotion recognition and regulation are core mechanisms of human functioning and mental health: Individuals need to be capable of sensing, labelling, and adjusting their emotional states to act adaptively in work, family, and clinical contexts (Damasio, 1999; Khalsa et al., 2018; Paulus & Stein, 2010). In Swiss clinical and working populations, including individuals with burnout or stress-related symptoms, patients with anxiety and depressive disorders, autistic adults, and increasingly also health professionals, this core capacity is often dysregulated: emotions can be experienced primarily as diffuse bodily sensations (e.g. “pressure on the chest,” “tight throat”) that are difficult to identify, express and regulate (Barkow et al., 2004; Nummenmaa et al., 2014; Bird & Cook, 2013). Such “bodily but wordless” emotions are reliably linked to higher exhaustion, cynicism and reduced efficacy at work (Cohen et al., 2023).

At the same time, the Swiss mental-health system shows structural frictions: 26 cantonal systems work without a coordinated stepped-care model, access varies by residence, and current financing still privileges inpatient settings over flexible outpatient or intermediate formats (Stulz et al., 2023; Richter et al., 2025; Stocker et al., 2016). This produces what federal reports call” underuse for severely ill people and overuse for mildly ill ones” (Richter et al., 2025). Digital, self-guided interventions are an innovative and alternative way to enhance access “between” therapist sessions for people who live with limited financial resources, timely flexibility, worry about privacy and stigma, or mobility. Yet most commercial mental-health products remain completely therapist-dependent and do not provide embodied, non-verbal ways to engage with emotions and regulation (Castro et al., 2024). Bodily Sensation Mapping (BSM) is a new, promising alternative. It builds on robust evidence that emotions are felt in distinct, cross-cultural body patterns (Nummenmaa et al., 2014). BMS is furthermore promising because it offers a non-verbal, embodied route into emotion work and therefore does not rely solely on talk-based therapy or high verbal/emotional literacy. What is still lacking, however, is (i) the inclusion of a third, clinically salient axis capturing constriction/pressure (the vocabulary patients actually use in anxiety, trauma, and stress contexts (Austin et al., 2006; Schlosberg, 1954)) and (ii) an embodied Virtual Reality (VR) implementation that allows individuals to inhabit a full-body avatar and “paint” felt sensations in 3D. The proposed project provides this extension: a three-axis, VR-based BSM coupled with real-time EMG/HRV so that it can be used autonomously by clients/patients and, at the same time, serve as an innovative, standardized add-on for psychotherapists working with body-oriented methods. In doing so, we translate a promising but therapist-dependent concept into a scalable, technology-supported intervention.
